No consensus has been reached regarding the best occlusal scheme for making complete dentures. Thus, the purpose of this systematic review was to compare bilateral balanced occlusion (BBO) with other occlusal schemes (canine guidance, lingualised occlusion and zero degree) in complete dentures. The schemes were compared in terms of quality of life/satisfaction and masticatory performance. Two independent reviewers performed a comprehensive search of studies published in or before October 2017 using the PubMed/MEDLINE, Scopus and Cochrane Library databases. The search was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. The focused question was: "In conventional complete denture, is BBO better than lingualised occlusion, canine guidance and zero degree in terms of quality of life, patient satisfaction and masticatory performance/muscle activity?" Seventeen studies were selected for analysis. In total, there were 492 patients with a mean age of 64.78 years and a mean follow-up duration of 2.96 months (range: 1-6 months). All studies compared BBO with the other occlusal schemes. Eleven studies evaluated the influence of the occlusal scheme designs on quality of life and satisfaction, and 8 studies evaluated masticatory performance and muscle activity between BBO and the other occlusion schemes. The present systematic review indicated that BBO does not confer better quality of life/satisfaction or masticatory performance and muscle activity. Thus, lingualised occlusion can be considered a predictable occlusal scheme for complete dentures in terms of quality of life/satisfaction and masticatory performance, while canine guidance can be used to reduce muscular activity.
Problems with dentures may arise because patients are not used to having something in their mouth that is not food. The brain senses the appliance and interprets it as 'food', sending messages to the salivary glands to produce more saliva and to secrete it at a higher rate. This usually only happens in the first 12 to 24 hours, after which the salivary glands return to their normal output. New dentures can also be the cause of sore spots as they compress the denture-bearing soft tissues (mucosa). A few denture adjustments in the days following insertion of the dentures can take care of this problem. Gagging is another problem encountered by a minority of patients. At times, this may be due to a denture that is too loose, too thick or extended too far posteriorly onto the soft palate. At times, gagging may also be attributed to psychological denial of the denture. Psychological gagging is the most difficult to treat since it is out of the dentist's control. Full arch dentures take up more space in the mouth than your normal teeth. Because of this, there is an adjustment period of time where you may notice difficulty in pronouncing certain words or talking in general. As dentures get re-adjusted and re-fitted, you may notice a difference in pronunciation and will have to make minor speech adjustments again.

I had a really great experience with Affordable Dentures and Implants. My mom has always been afraid of dentists and didn't take care of her teeth. She also had a stroke a few years back which has created a few more barriers to treatment. It's difficult to explain to anyone who doesn't personally know a stroke survivor, but she gets antsy pretty quickly and can be a bit difficult to work with at times. She's also physically handicapped and can't use her left side, plus is having trouble transferring from her wheelchair to other seats, getting into a helpful position, and following directions, so it's a bit of a unique situation. She underwent major oral surgery in this office- multiple extractions and bone augmentation- in order to get dentures. This is something we've seen a couple different dentists for, but she has always refused treatment because she didn't like something about the office or wanted to be put to sleep in a hospital, which was out of our budget. Given her limitations and dental anxiety, I wasn't sure what to expect this time around, but I couldn't have been happier with how this office behaved. The front office has been great in terms of explaining processes, scheduling, and billing. I've also had to coordinate paratransit to get my mom to the office, and they've worked with me on times to make sure everything lines up right. Something you can probably only appreciate if you've been in a similar situation. They've also worked with us on fixing issues and missed appointments due to unforeseeable circumstances. Not only did they not scold us for missing an appointment, but they worked with me to resolve the problem that caused it- my mom's assisted living facility biffed it and didn't give her pain medicine after her surgery. :/ But, like I said, this office came through and helped- always making her care the top priority. As far as back office goes, Carlos the assistant has the patience of a saint. Seriously. We had to do a full series of x-rays because my mom couldn't stand up for the pano. My mom had trouble holding still and repeatedly failed to follow instructions. Carlos never stopped smiling and encouraging her. He had a lot of positive energy and kept joking with her throughout, which I know went a long way to making her feel good about the office. He did the same during the other visits, but the x-rays I'm sure were the biggest challenge. Jennifer assisted with the surgery. She was incredibly kind, gentle, and thoughtful. She was always doing what she could to keep my mom comfortable and looking out for her. She even went and grabbed my mom a pillow to make sure she was comfortable. Dr. Athari has an amazing sense of calm about him. He walked us through everything and talked to us throughout the procedure. My mom needed frequent breaks during the surgery and he always gave her the time she needed to regroup. He was very gentle and mindful with her throughout the procedure, and I'm certain that's what got my mom through it without issue. I cannot stress how difficult this process could have been considering the amount of care needed and the limitations of the patient. However, these folks rolled with everything we threw at them and never missed a beat. Always helpful, knowledgeable, kind, and gentle. If I had to find one negative about the office, it would be the wait times. On our first visit, one woman in the waiting room said she'd been waiting 30+ minutes. It took a day to get a call back to schedule. We waited a little while in the office too. However, when you consider that they're working with people like the person I brought in, and that it's totally impossible to predict the needs and behaviors of some of the patients who go in there, it's totally understandable. I have no doubt they took a little longer because they were treating someone else exactly as they treated us- with patience and understanding. That's worth waiting for. So, if you do go, be prepared for some short delays, but also know why those delays happen. Overall, they work like a well-oiled machine. We got to meet the whole team, including their on-site lab techs. Every single person met us with a smile and kind words. I cannot thank Dr. Athari enough for creating an environment like this. Been struggling with getting my dentures since June. After waiting weeks for first appointment, came back for second appointment. Was told can't complete services because location was then closing for a week because first doc was fired. Asked me to reschedule at least a week after reopening since they'd need to catch up. So I rearranged my schedule. OK. Came back yesterday, Thursday, and the girl dropped my impression on the floor right in front of me, then told me to come back at 1:00. I asked if needed to redo the impression but she just brushed it off. As soon as I was 10 miles away, I was called back to redo the impression because they needed a better one. OK. So I return and make another impression, and left with pain, bleeding, and swelling. Told to return at 1:30 to get my new teeth. OK.
There are three main ways to replace missing teeth. The first is with a removable false tooth (or teeth) - called a partial denture. The second is with a fixed bridge. This type of restoration is less expensive but doesn’t look as natural or function as well as an implant-supported crowns. I at age 16 was in car accident went through the windshield and lost 4 on top and 4 lower so have had partials the last 30 years. Now I have one tooth left on top so I at least need the upper denture implant as for the bottoms well they are not great but can make due a few more years before I will have to get them. I have very lil income as I have just started my cleaning business last year. That being said I am very willing to trade services so long as it a denture implant or something very similar. Right now business is pretty slow always is during the summer. I am OCD and a perfectionist most likely the reason I love cleaning and do it better than most anyone with lil to no chemicals except the normal items you already have at home that care not harmful to the environment people/pets. You give me a beautiful smile back so I can also eat more than soups/foods that require lil to no chewing. I will give u the cleanest office & home 5 days a week for 6-8 months promise
When dentures no longer fit well, the correct action is to seek follow-up care. Denture adhesive is recommended for dentures, even well fitting ones. Adhesives improve retention, seal the borders to prevent food from slipping under, and cusion the soft tissues from the rigid acrylic. Using denture adhesive may improve the fit, but it tends to work best when only a small amount is used. Covering the entire denture fitting surface in adhesive makes it stay in less well. Adhesives may compensate for gradual loosening of a denture, but it is only a temporary solution; it does not solve the problem. Fortunately, dentures can often be relined with relining materials to improve the fit, and this process costs less than creation of new dentures. Overall, a well-made denture could last about 5 years (or more), but this is different for every patient.

After deciding on having the extractions and the full plate, I decided on the "Premium" denture. You can see 3 different types of dentures. Economy, Good and Premium. It's pretty easy, if you want the longest lasting, then get the premium. Economy dentures often break after a couple years... I had to pay the full amount right then, before they would go any further. They want all the money in advance or they don't do the dentures. Be prepared to pay up front in full, before you go, or don't go.
FRANKLIN, INDIANA -- On June 19 and 20, 2013, I visited Affordable Dentures, Franklin, Indiana, to receive same-day partials. From the clinic published offerings, I selected what is termed "The Premium Package," at a cost of $1780. The Package consists of a set of full or partial dentures, with the promise of being allowed to return at any time during the first year of wear for repair and adjustments. A final set is then placed in the patient's mouth. The words "at any time" echo throughout the clinic as the patients are being fitted for teeth, and again, as they leave: "You may come back any time you need to." That promise turns out to be a lie, particularly for purchasers of The Package.
Remember: You can do serious harm to your denture and to your health by trying to adjust or repair your denture. Using a do-it-yourself kit can damage the appliance beyond repair. Glues sold over-the-counter often contain harmful chemicals and should not be used on a denture. If your denture breaks, cracks or chips, or if one of the teeth becomes loose, call your dentist immediately. In many cases, dentists can make necessary adjustments or repairs, often on the same day. Complicated repairs may require that the denture be sent to a special dental laboratory.
I will ask to see the dentist when I return and tell him that if the permanent denture is not going to be formed with any more care than this, I want the remainder of my money back and I will go elsewhere. Even for an "affordable denture" the quality of this temporary is not worth the mouth pain and I will never be able to wear it for 6mo to 1yr while I wait for the permanent one. If you can afford better, then please do, as you will not be happy with the poor quality results.

I had partials made in apri they were awful I was ordered a new top I can not wear the bottoms then hurt to much the top partial is like chewing cardboard I would never consider going back again it cost me 1700.00 dolllars that I am still paying for I am on as and this is reall a burden to pay this every month for something I can not wear. This was one of the best and most helpful Dentists that I have been to in my life. I came in for a free x-ray and consultation appointment only with hopes of getting work done soon. Unfortunately I developed an abscess the day before my appointment. Well when I got there (Moyock, NC) they noticed I was in pain and decided I needed help ASAP so after the xray they offered to take care of the abscess and tooth! They worked very quickly and gave me a denture, all for a really awesome price. Highly recommended to anyone in need of any extraction or denture.
Most new denture issues are caused by gums adjusting to extractions and the placing of artificial teeth. These are not problems particular to me. My gums were ready for permanent partials, yet Affordable Dentures refused to provide them. At one point, Dr. **, in an effort to deflect responsibility for her inferior work, pointed to my natural tooth #22, and said, "It's this one that's causing all the trouble. This tooth gave us FITS!" Tooth #22 had nothing to do with the creation or placement of her shoddy partials. My #22 is perfectly normal and was not in the way of anything. Dr.** simply had to divert attention from her fraud by projecting away from herself.
Apppointment was at 10 am got here at 9 am. Then we finally got back there at 12 pm, and then the people say come back at 3pm and we will bring you straight to the back. Then we got back around 2pm and then we are still waiting, and they just said it would be another hour. This is by far the worst dentist office I have ever been to... Very disappointing.

Generally speaking partial dentures tend to be held in place by the presence of the remaining natural teeth and complete dentures tend to rely on muscular co-ordination and limited suction to stay in place. The maxilla very commonly has more favorable denture bearing anatomy as the ridge tends to be well formed and there is a larger area on the palate for suction to retain the denture. Conversely, the mandible tends to make lower dentures much less retentive due to the displacing presence of the tongue and the higher rate of resorption, frequently leading to significantly resorbed lower ridges. Disto-lingual regions tend to offer retention even in highly resorbed mandibles, and extension of the flange into these regions tends to produce a more retentive lower denture. An implant supported lower denture is another option for improving retention.

Overdentures are an alternative that can be used if traditional dentures prove to be extremely uncomfortable or if you have a few natural teeth left. Overdentures are fitted over the roots of natural teeth and either rest on these or on dental implants, if there are no natural teeth to fit over. Some find this type of denture more comfortable and they are also easily removable.
