This type of denture is made to be placed in the mouth immediately after you have all your natural teeth extracted. This allows you to leave the office without the embarrassment of having no teeth while the gums heal from the extraction. The denture is made to conform to your mouth at the moment impressions were taken. Your gums will change drastically over the next few months and it is possible that the dentures will rub against the gums causing some soreness until the denture is adjusted. Most patients will require a realignment of their denture within the first few months due to the gums shrinking from the extraction of teeth. Another benefit of immediate dentures is the fact that the dentures act as a bandage to the extraction sites which covers the tooth sockets and prevents them from becoming irritated. Just remember, never remove the denture yourself, even for a brief moment unless your dentist has instructed you to. The gums have a tendency to swell when uncovered at first; and if you are without your dentures for a little while, they may never go back in.
After implant-supported dentures are in place, patients should properly care for their teeth and gums. The dentures, gums, and tongue should be brushed twice a day. Patients should also floss daily to remove debris between the teeth; though false teeth won't become damaged by decay, plaque build-up can still cause gum disease and bad breath. Hypochlorite cleansers have a disinfectant action and they remove non-viable organisms and other deposits from the surface, but they are not very good at eliminating calculus from the denture surface. Immersing dentures in a hypochlorite solution for more than 6 hours occasionally will eliminate plaque and heavy staining of the denture. Furthermore, as microbial invasion is prevented, the deterioration of the soft lining material does not occur.[32] Although, corrosion of cobalt chromium has occurred when hypochlorite cleansers have been used and they may also result in the fading of the acrylic and silicone lining, but the softness or elastically of the linings are not greatly changed.[35]
The process of fabricating a denture usually begins with an initial dental impression of the maxillary and mandibular ridges. Standard impression materials are used during the process. The initial impression is used to create a simple stone model that represents the maxillary and mandibular arches of the patient's mouth. This is not a detailed impression at this stage. Once the initial impression is taken, the stone model is used to create a 'Custom Impression Tray' which is used to take a second and much more detailed and accurate impression of the patient's maxillary and mandibular ridges. Polyvinylsiloxane impression material is one of several very accurate impression materials used when the final impression is taken of the maxillary and mandibular ridges. A wax rim is fabricated to assist the dentist or denturist in establishing the vertical dimension of occlusion. After this, a bite registration is created to marry the position of one arch to the other.
Powder application. Sprinkle a thin, uniform layer throughout the tissue-bearing surface of the denture. Shake off excess powder and press the denture into place. Powders may be preferred over pastes because they are easier to clean off the denture and tissue. In addition, they don't have the same tendency as pastes do to "shim" (keep the denture away from the tissue).
