Generally speaking partial dentures tend to be held in place by the presence of the remaining natural teeth and complete dentures tend to rely on muscular co-ordination and limited suction to stay in place. The maxilla very commonly has more favorable denture bearing anatomy as the ridge tends to be well formed and there is a larger area on the palate for suction to retain the denture. Conversely, the mandible tends to make lower dentures much less retentive due to the displacing presence of the tongue and the higher rate of resorption, frequently leading to significantly resorbed lower ridges. Disto-lingual regions tend to offer retention even in highly resorbed mandibles, and extension of the flange into these regions tends to produce a more retentive lower denture. An implant supported lower denture is another option for improving retention.

Thus, the purpose of this systematic review was to compare bilateral balanced occlusion (BBO) with other occlusal schemes (canine guidance, lingualised occlusion and zero degree) in complete dentures. The schemes were compared in terms of quality of life/satisfaction and masticatory performance. Two independent reviewers performed a comprehensive search of studies published in or before October 2017 using the PubMed/MEDLINE, Scopus and Cochrane Library databases. The search was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. The focused question was: "In conventional complete denture, is BBO better than lingualised occlusion, canine guidance and zero degree in terms of quality of life, patient satisfaction and masticatory performance/muscle activity?" Seventeen studies were selected for analysis. In total, there were 492 patients with a mean age of 64.78 years and a mean follow-up duration of 2.96 months (range: 1-6 months). All studies compared BBO with the other occlusal schemes. Eleven studies evaluated the influence of the occlusal scheme designs on quality of life and satisfaction, and 8 studies evaluated masticatory performance and muscle activity between BBO and the other occlusion schemes. The present systematic review indicated that BBO does not confer better quality of life/satisfaction or masticatory performance and muscle activity. Thus, lingualised occlusion can be considered a predictable occlusal scheme for complete dentures in terms of quality of life/satisfaction and masticatory performance, while canine guidance can be used to reduce muscular activity.
I had 16 teeth pulled and plates made for under $1,000. The initial appointment started with a consultation. I didn't actually have anything done that day. The dentist and an assistant came in and gave me the costs as well as explained the process. dr recommended immediate dentures. what I read on immediate dentures was that they are generally temporary. apparently per this dentist not so. now wants to charge 380 for reline knowing the bone was receded a lot and would have to reline right away. given all I know now, I would never go to this dentist again. ask more questions, get more estimates from other dentists. upper plate only has 12 teeth in it. inside of teeth do not touch lower permanent teeth, gap of at least 1/8 " between gm and inside of plate, gum does not touch bottom of gums at all.

My heart doctor said that I could not go off meds but could get tooth pulled. I wasted 4 hours of my time today to be denied service I requested. Apparently the dentist is too worried about being sued (a statement she made to me about her not wanting to be sued if I changed my mind after the fact - you'd think they'd have some kind of legal waiver for patients like me) to consider the fact that I'm in extreme pain, have limited resources, have spent 30 years and tens of thousands of dollars to fix the teeth that she wants kept in my mouth and I want removed. The impact of my current condition is both physical and emotional. However, to her credit she said it was in my best interest not to get dentures and was not charged for the visit. I'm irritated that my opinion didn't count at all and was flat out denied service I wanted.

Conventional full dentures are used to replace a full set of teeth either on the upper or lower jaw. When all of the patient’s natural teeth have been removed and the gum tissues have healed, conventional full dentures may be placed in the mouth. In between, the patient may be fitted with an immediate denture that can be inserted on the same day the teeth are extracted. Using immediate full dentures is beneficial for the patient because this means that he does not have to go without teeth for a certain period. Readjustment of the dentures will have to be made after several months because the supporting bone will reshape as it heals and this will cause the denture to loosen.

To minimize staining, properly clean your dentures daily to remove food and plaque bacteria. Moisten the brush and apply a nonabrasive denture paste (regular toothpaste is too abrasive) or use liquid soap. Brush every surface, inside and out, scrubbing gently. A variety of over-the-counter denture-cleanser products may be safely used (by following the manufacturer’s instructions) to remove some stains. You may also consider soaking your dentures in a cup of water with a teaspoon of household bleach, but be sure to rinse them off well before putting them back in your mouth. When cleaning your dentures, be careful not to drop them as they can break. You should clean your dentures over a sink that is filled with water or has a washcloth in place to prevent the dentures from breaking should you drop them while cleaning. Dentures are generally worn during the day and taken out at night to give the oral tissues time to relax. During the first few days after receiving the denture, however, it needs to remain in the mouth even when sleeping to best identify areas that need to be adjusted. This is especially important after receiving an immediate denture, for the gum tissues will swell after the teeth are extracted or lost and then may not permit the denture to be reinserted if taken out.

The maxillary arch receives primary support from the horizontal hard palate and the posterior alveolar ridge crest. The larger the denture flanges (that part of the denture that extends into the vestibule), the better the stability (another parameter to assess fit of a complete denture). Long flanges beyond the functional depth of the sulcus are a common error in denture construction, often (but not always) leading to movement in function, and ulcerations (denture sore spots).
