Removable partial dentures usually consist of replacement teeth attached to pink or gum-colored plastic bases. Depending on your needs, your dentist will design a partial denture for you. A partial denture may have a metal framework and clasps that connect to your teeth, or they can have other connectors that are more natural looking. In some cases, a removable partial denture is made to attach to your natural teeth with devices called precision attachments. Precision attachments are generally more esthetic than clasps.
Your dentist or prosthodontist will instruct you as to how long to wear dentures and when to remove them. During the first several days after receiving your denture, you may be asked to wear it all the time, including while you sleep. Although this may be temporarily uncomfortable, it is the quickest way to identify the areas on the denture that may need adjustment. When dentures no longer fit well, the correct action is to seek follow-up care. Denture adhesive is recommended for dentures, even well fitting ones. Adhesives improve retention, seal the borders to prevent food from slipping under, and cusion the soft tissues from the rigid acrylic. Using denture adhesive may improve the fit, but it tends to work best when only a small amount is used. Covering the entire denture fitting surface in adhesive makes it stay in less well. Adhesives may compensate for gradual loosening of a denture, but it is only a temporary solution; it does not solve the problem. When are Dentures Necessary?What are Temporary Partial Dentures?If you’ve had teeth removed due to decay or gum disease and will be receiving permanent dentures, your dentist may suggest temporary or “immediate” dentures. These are dentures you can wear for the first two to three months immediately after tooth removal. They can be especially helpful for someone with a history of sensitive teeth and gums, since the patient does not have to be without teeth while the area heals, and any remaining sensitive teeth are subject to less pressure from chewing food.The creation of temporary dentures may require four to five dental visits prior to tooth extraction. Temporary dentures are made using basically the same technique as a permanent denture, but because they are made in advance of any surgery, you can’t tell exactly how your mouth will look after teeth are removed. By contrast, the molds for conventional dentures are made once the gum tissues have healed following any tooth extractions, which takes about six to eight weeks. If you opt for a dental bridge, your dentist will fit customized crowns to the teeth on either side of the gap to be bridged. A false tooth (or teeth) called a pontic is attached to the crowns and replaces your missing tooth (or teeth).Depending on your oral health care needs, you can choose partial dentures that are removable, but bridges are permanently cemented into place. There are three main types of bridges to suit a variety of dental restoration situations:Traditional: This style involves placing crowns on the teeth that border the missing teeth.Cantilever: This style is used if you only have teeth on one side of the missing tooth or teeth.Maryland bonded: This type of bridge consists of a porcelain tooth (or teeth) in a metal framework, with wings to attach it to existing teeth.Keeping the surrounding, supporting teeth healthy is essential to get the maximum benefit from a dental bridge, so be sure to follow a regular oral care routine of twice-daily tooth brushing and daily flossing. Other denture cleaning methods include enzymes, ultrasonic cleansers and microwave exposure.[32] A Cochrane Review found that there is weak evidence to support soaking dentures in effervescent tablets or in enzymatic solutions and while the most effective method for eliminating plaque is not clear, the review shows that brushing with paste eliminates microbial plaque better than inactive methods. There is a need for studies to provide reports about the cost of materials and the negative effects that may be associated with their use as these factors could affect the acceptability of such materials by patients which will in turn affect their effectiveness in a daily setting in the long term. Additionally putting dentures into a dishwasher overnight can be a hndy short cut when away from home. Additionally, further studies comparing the different methods of cleaning dentures are needed.[39]
In cases where teeth need to be removed, an immediate denture is typically placed to enable proper healing of the extraction sites and serve as an esthetic replacement for natural teeth. This can be easily modified for changing ridge contours during healing until final dentures can be made. In constructing the immediate dentures, dentists will use a shade and mold chart to choose replacement teeth that will most closely match your natural teeth, minimizing any changes in appearance.
Hypochlorite cleansers have a disinfectant action and they remove non-viable organisms and other deposits from the surface, but they are not very good at eliminating calculus from the denture surface. Immersing dentures in a hypochlorite solution for more than 6 hours occasionally will eliminate plaque and heavy staining of the denture. Furthermore, as microbial invasion is prevented, the deterioration of the soft lining material does not occur.[32] Although, corrosion of cobalt chromium has occurred when hypochlorite cleansers have been used and they may also result in the fading of the acrylic and silicone lining, but the softness or elastically of the linings are not greatly changed.[35]

Deposits such as microbial plaque, calculus and food debris can accumulate on the dentures, which may lead to issues such as angular stomatitis, denture stomatitis, undesirable odours and tastes as well as staining. The deposits can also quicken the rate at which some of the denture materials wear down.[26] Due to the presence of these deposits, there is an increased risk of the denture wearer and other people around them developing a systemic disease by organisms such as methicillin-resistant Staphylococcus aureus (MRSA),[27] but research shows that denture cleaners are effective against MRSA.[28][29][30] Therefore, denture cleaning is imperative for the overall health of the denture wearers as well as for the health of people they come into contact with.[31]
High-end dentures usually involve a great measure of dental artistry and utilize materials which simulate the natural look and color of teeth and gums to the highest degree possible. The teeth, normally made from composite acrylic resins, are designed to last a long time and often include a warranty against wear and tear, such as chipping and cracking. Like a bridge, partial dentures rest on surrounding teeth to fill in the gaps where one or more teeth are missing. But unlike a bridge, partial dentures are fully removable by the wearer. Partials are affordable alternatives to other types of dental prosthetics and are custom-made to blend in with each patient’s natural teeth. It takes a little time to adapt to new partials, but many people find that they reclaim much of their original function and aesthetics with partial dentures in place.
Problems with dentures may arise because patients are not used to having something in their mouth that is not food. The brain senses the appliance and interprets it as 'food', sending messages to the salivary glands to produce more saliva and to secrete it at a higher rate. This usually only happens in the first 12 to 24 hours, after which the salivary glands return to their normal output. New dentures can also be the cause of sore spots as they compress the denture-bearing soft tissues (mucosa). A few denture adjustments in the days following insertion of the dentures can take care of this problem. Gagging is another problem encountered by a minority of patients. At times, this may be due to a denture that is too loose, too thick or extended too far posteriorly onto the soft palate. At times, gagging may also be attributed to psychological denial of the denture. Psychological gagging is the most difficult to treat since it is out of the dentist's control. In such cases, an implant-supported palateless denture may have to be constructed. Sometimes there could be a gingivitis infection under the completed dentures, caused by the accumulation of dental plaque. One of the most common problems for wearers of new upper complete denture is a loss of taste sensations.

Patients should combine the brushing of their dentures with soaking them in an immersion cleaner from time to time as this combined cleaning strategy has been shown to control denture plaque.[33] Due to microbial invasion, the lack of use of immersion cleaners and inadequate denture plaque control will cause rapid deterioration of the soft linings of the denture.[34]
Some partial dentures are made almost entirely of acrylic – but just like metal framework dentures, acrylic dentures have wire clasps that attach to any remaining natural teeth. These are generally used as a cost-effective alternative to metal-based partial dentures. They can also be used for transitional dentures when you’re expected to have additional prosthetic treatments, such as implants, in the future.
Dentures are mainly made from acrylic due to the ease of material manipulation and likeness to intra-oral tissues, ie. gums. Most dentures made are fabricated from heat-cured acrylic polymethyl methacrylate and rubber-reinforced polymethyl methacrylate.[3] Coloring agents and synthetic fibers are added to obtain the tissue-like shade, and to mimic the small capillaries of the oral mucosa, respectively.[4] However, dentures made from acrylic can be fragile and fracture easily if the patient has trouble adapting neuromuscular control. This can be overcome by reinforcing the denture base with cobalt chromium (Co-Cr). They are often thinner (therefore more comfortable) and stronger (to prevent repeating fractures).
As far as back office goes, Carlos the assistant has the patience of a saint. Seriously. We had to do a full series of x-rays because my mom couldn't stand up for the pano. My mom had trouble holding still and repeatedly failed to follow instructions. Carlos never stopped smiling and encouraging her. He had a lot of positive energy and kept joking with her throughout, which I know went a long way to making her feel good about the office. He did the same during the other visits, but the x-rays I'm sure were the biggest challenge.

New dentures may feel awkward for a few weeks until you become accustomed to them. The dentures may feel loose while the muscles of your cheek and tongue learn to keep them in place. It is not unusual to experience minor irritation or soreness. You may find that saliva flow temporarily increases. As your mouth becomes accustomed to the dentures, these problems should go away. Follow-up appointments with the dentist are generally needed after a denture is inserted so the fit can be checked and adjusted.
