No consensus has been reached regarding the best occlusal scheme for making complete dentures. Thus, the purpose of this systematic review was to compare bilateral balanced occlusion (BBO) with other occlusal schemes (canine guidance, lingualised occlusion and zero degree) in complete dentures. The schemes were compared in terms of quality of life/satisfaction and masticatory performance. Two independent reviewers performed a comprehensive search of studies published in or before October 2017 using the PubMed/MEDLINE, Scopus and Cochrane Library databases. The search was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. The focused question was: "In conventional complete denture, is BBO better than lingualised occlusion, canine guidance and zero degree in terms of quality of life, patient satisfaction and masticatory performance/muscle activity?" Seventeen studies were selected for analysis. In total, there were 492 patients with a mean age of 64.78 years and a mean follow-up duration of 2.96 months (range: 1-6 months). All studies compared BBO with the other occlusal schemes. Eleven studies evaluated the influence of the occlusal scheme designs on quality of life and satisfaction, and 8 studies evaluated masticatory performance and muscle activity between BBO and the other occlusion schemes. The present systematic review indicated that BBO does not confer better quality of life/satisfaction or masticatory performance and muscle activity. Thus, lingualised occlusion can be considered a predictable occlusal scheme for complete dentures in terms of quality of life/satisfaction and masticatory performance, while canine guidance can be used to reduce muscular activity.

This arrangement of teeth is tested in the mouth so that adjustments can be made to the occlusion. After the occlusion has been verified by the dentist or denturist and the patient, and all phonetic requirements are met, the denture is processed.

Generally speaking partial dentures tend to be held in place by the presence of the remaining natural teeth and complete dentures tend to rely on muscular co-ordination and limited suction to stay in place. The maxilla very commonly has more favorable denture bearing anatomy as the ridge tends to be well formed and there is a larger area on the palate for suction to retain the denture. Conversely, the mandible tends to make lower dentures much less retentive due to the displacing presence of the tongue and the higher rate of resorption, frequently leading to significantly resorbed lower ridges. Disto-lingual regions tend to offer retention even in highly resorbed mandibles, and extension of the flange into these regions tends to produce a more retentive lower denture. An implant supported lower denture is another option for improving retention.
If it was possible I would give them an absolute minus 10 Stars. First off I go in the office. The staff that greeted me was polite that I will give them. Had X-rays done. Was waiting to see what the doctor would say. When he came in he had no bedside manner. I was leaning back in a chair and shook his hand and he said he would try to save a couple of teeth whatever. Was talking for a little bit and then walked out of the room. I don't know if it was an assistant I assume was telling me options I had. Then the doctor Vick walked in and I thought he was joking at first. He asked me why I squeeze his hand so hard when I shook it. Mind you the hand I shook with has been broken and operated on before and still has issues.
You should see your dentist 24 hours after delivery of your new dentures. It is not unusual to experience some initial discomfort. Minor adjustments can increase comfort and eliminate problems before they become more serious. Initially, a new denture may feel unusual in the mouth. Getting 16 teeth pulled is about as pleasant as it sounds. I have had many teeth extracted in the past and this was by far the most unpleasant experience I have had. The most curious thing about this visit was there was NO assistant involved in the actual dental work. The dentist was working alone for the entire time. So I wound up with my partial getting a tooth put in it as I thought they would pull my tooth but when the doctor's fax came, it still was not good enough for them. I believe that he just did not want to service me by pulling my tooth. I have a partial I cannot wear because the tooth put in there it will not fit in my mouth. I informed the front desk of needing to get to work. I let the young woman in back know as well. I understood having to wait an hour for the test fitting of the impression. Well without any concern for my time the mold apparently was not handled properly and was broken. Now I have to wait until 2 pm? I was to be at work by noon after informing them I'd be late. Never at Mesquite location again! We have used three other locations without any issues now this and my son had similar treatment!
The denture itself is not very good. Again, I fault this due to a low paid lab worker, who is probably texting while he makes the denture. It clearly did not fit properly, too much in one area, too little in another. I am glad that I have had a partial for years and an upper for years, as I know what can be done to alleviate the pressure points. I had to remove material in several spots on the lower edges of the denture, to keep it from bruising my gums.
The dentist let the receptionist girl put the goo in the ID that's sets to a clear rubber to cushion between the ID & my newly extracted teeth. It had too much on one side, was crooked in my mouth & at the top on the other side had no cushion lining which caused a bad sore to form almost immediately. After 5 days I couldn't take it anymore. I drove there (55 miles) to get them to redo the lining. The first time it took her all of 5 minutes to do it. I walked in, they were all there... but was informed that they were ‘about' to leave & told I had to come back the next day! No compassion or good customer care for their patients. DO NOT GO HERE!

Denture adhesives are not, however, a remedy for ill-fitting dentures. The next week I came in early in the morning to have my impressions made. This was fairly quick and I was only in the office for about an hour total. Needless to say that I am going to a different dentist. A man who has been in this community for a long time and has a spotless office with people who treat their patients with respect. Charges more, but worth it to be treated right. I have gone to Affordable Dentures for a while because of the pricing but every time I went, new doctor, new office people, but these people at my last time there, they are really nasty. I am also going to contact the board of health because while I was sitting in the dental chair I looked up at the light and it was filthy, nasty with what looked like old spit or blood or whatever it was. It was not clean in my opinion.

Later that afternoon I came back to have 16 teeth extracted. I got to the office and waited about 30 minutes before I was seen. I got to the back and began injections. After this I went to a full blown panic attack and came close to passing out. The one positive is the dentist who was working on me took it in stride. I wasn't treated disrespectfully and after about 30 minutes the panic subsided and she was able to finish my injections and begin pulling teeth.
